About The Position

Pleasant Hill Post Acute is seeking a compassionate, motivated, and dedicated Occupational Therapist to join our rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunities for OTs who are passionate about helping patients regain independence and achieve their highest level of function. Our rehabilitation program emphasizes quality patient care, teamwork, communication, and collaboration across nursing, therapy, and other departments.

Requirements

Current California Occupational Therapist (OT) license in good standing.
Strong clinical, communication, and organizational skills.
Ability to work effectively as part of an interdisciplinary team.
A positive attitude and genuine commitment to patient care.

Nice To Haves

Skilled nursing or post-acute rehabilitation experience preferred.
New graduates are welcome to apply.

Responsibilities

Evaluate patients and develop individualized treatment plans based on their functional needs and goals.
Provide skilled occupational therapy services to help patients improve independence with activities of daily living.
Help patients maximize functional abilities and improve their overall quality of life.
Monitor progress and adjust treatment plans as appropriate.
Collaborate with physicians, nursing staff, physical therapists, speech therapists, and other members of the interdisciplinary team.
Communicate effectively with patients, families, and caregivers regarding treatment goals and progress.
Complete accurate, timely, and compliant clinical documentation.
Provide compassionate, patient-centered care while maintaining professional standards.
