Occupational Therapist - Joliet, IL
About The Position
FOX Rehabilitation is hiring Occupational Therapists to provide one-on-one care to older adults in Joliet, Illinois, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care. Clinicians use FOX’s Geriatric House Calls™ model to help patients improve mobility, functional independence, wellness, and quality of life. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule, develop lasting patient relationships, and work within a collaborative, interdisciplinary team. FOX describes its practice as clinician-operated and grounded in meaningful relationships, evidence-based care, and a mission to abolish ageism. Employment pathways include full-time, part-time, and PRN/FOX Flex positions; compensation and benefits depend on employment type and eligibility. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; providing moderate to maximum assistance with patient transfers and bed mobility; repositioning patients; standing and walking for extended periods; climbing stairs; performing therapeutic activities; and wearing PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living communities.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Conduct therapy sessions supporting patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or other virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
