About The Position

Puget Sound Home Health & Hospice is seeking a compassionate Occupational Therapist (OT) to join our Kitsap County Home Health team. You'll help patients regain independence, improve safety, and accomplish the everyday activities that matter most to them, all in the comfort of home. This role offers more time with patients and more autonomy in your day, allowing you to focus on one person at a time.

Requirements

Current Washington State Occupational Therapist license, or eligible for licensure before your first day
1–2 years of Occupational Therapy experience
Current CPR certification
Ability to work independently while collaborating with an interdisciplinary team

Nice To Haves

Home Health or community-based experience preferred

Responsibilities

Assess patients' functional abilities, limitations, and home environment
Develop individualized treatment plans focused on independence and meaningful daily activities
Provide skilled OT interventions to improve mobility, strength, coordination, safety, and activities of daily living
Evaluate home safety and recommend adaptive equipment or modifications when appropriate
Educate patients, families, and caregivers on techniques that support independence and safety
Collaborate with nurses, therapists, providers, and other members of the interdisciplinary care team
Monitor patient progress and adjust treatment as needs change
Complete accurate and timely clinical documentation
