Occupational Therapist (OT) - Skilled Nursing Rehabilitation

Concept Rehab is seeking a compassionate and motivated Occupational Therapist (OT) to join our rehabilitation team in a skilled nursing and post-acute care environment. Requirements
- Bachelor's, Master's, or Doctorate degree in Occupational Therapy
- Current Ohio Occupational Therapist license in good standing
- Skilled nursing, geriatric, or post-acute rehabilitation experience preferred
- Strong clinical reasoning and problem-solving abilities
- Excellent communication and interpersonal skills
- Commitment to patient-centered care and positive clinical outcomes

Responsibilities
- Perform comprehensive occupational therapy evaluations and functional assessments
- Develop individualized treatment plans based on patient goals and clinical needs
- Deliver skilled interventions focused on activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
- Address cognitive, perceptual, sensory, upper extremity, and functional mobility deficits
- Recommend adaptive equipment, environmental modifications, and safety strategies
- Educate patients, families, and caregivers to support successful outcomes
- Collaborate with nursing, physicians, rehabilitation professionals, and facility leadership
- Supervise and support Certified Occupational Therapy Assistants (COTAs) as appropriate
- Participate in interdisciplinary care plan meetings and discharge planning discussions
- Maintain accurate, timely, and compliant documentation in accordance with regulatory requirements
