About The Position

Home Sweet Home is seeking a compassionate Occupational Therapist (OT) to join our growing Home Health team in the West Valley, Arizona. Requirements

Current Arizona Occupational Therapist (OT) license
Graduate of an ACOTE-accredited Occupational Therapy program
Minimum one year of Occupational Therapy experience (Home Health preferred)
Strong communication, organization, and time management skills
Experience with EMR documentation
Valid driver's license, reliable transportation, and current auto insurance

Responsibilities

Complete in-home Occupational Therapy evaluations and reassessments
Develop individualized treatment plans to improve independence with Activities of Daily Living (ADLs)
Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization
Recommend adaptive equipment, home modifications, and fall prevention strategies
Educate patients and caregivers on safety, mobility, and functional independence
Collaborate with physicians, nurses, Physical Therapists (PT), Speech-Language Pathologists (SLP), and the interdisciplinary care team
Document patient progress accurately using EMR while maintaining Medicare and agency compliance
