About The Position

The Occupational Therapist will be responsible for evaluating the clinical needs of patients, developing and implementing care plans, and ensuring patient outcomes meet national benchmarks. This role involves collaborating with a multidisciplinary team, supervising Home Health Aides, and coordinating patient discharges. The therapist will also be responsible for accurate and timely documentation, obtaining physician orders, and referring patients to community resources as needed. A strong commitment to service excellence and VNA's CARES values (compassion, accountability, respect, excellence, and service) is expected.

Requirements

  • Bachelor of Science Degree or Master's Degree in Occupational Therapy from an institution accredited jointly by the American Occupational Therapy Association and the committee on Allied Health Education and Accreditation of the American Medical Association.
  • Current OT license in Mass.
  • Minimum one (1) year clinical experience.
  • CPR certification required.

Responsibilities

  • Maintain annual HHRG values at the National Standard as appropriate.
  • Utilize HHRG values to establish visit frequency and to establish appropriate interdisciplinary utilization.
  • Ensure annual outcomes meet National Benchmarks.
  • Refer to community resources as appropriate.
  • Evaluate the clinical needs of the patient; initiate, develop, implement and make necessary revisions to the patient's plan of care; address all problems in the plan of care and document accordingly.
  • Participate in inter-disciplinary patient case conferences; evaluate effectiveness of established plan of care.
  • Instruct and provide documented supervision of the Home Health Aide plan of care every 14 days.
  • Notify MD/other involved care providers of significant changes in the patient’s condition.
  • Complete admission and other paperwork on a timely basis.
  • Record narrative documentation to reflect skilled care. Documentation is clear, concise and addresses specifics of teaching, direct care and response of patient / caregiver.
  • Obtain and clarify valid MD orders for all care delivered.
  • Plan and coordinate discharges appropriately and inform patient/MD and record disposition of patient in the discharge summary. Complete all appropriate discharge paperwork.
  • Confer with COTA and provide written documentation regarding baseline data, treatment plan, patient status and precautions prior to initial COTA visit. Delegate patient care appropriately. Supervise and case conference with COTA according to departmental guidelines.
  • Instruct patient family, significant other, in patient's total Occupational Therapy treatment program.
  • Perform all other duties as assigned.
