Requirements

Bachelors of Science Degree in Occupational Therapy required.
Current State of CT Occupational Therapy license required.
Previous experience as an Occupational Therapist preferred.

Nice To Haves

Master’s Degree in Occupational Therapy or Doctor of Occupational Therapy preferred.

Responsibilities

Independently evaluates patients’ level of function and determines, develops, and implements individualized treatment goals and plans in alignment with patient/caregiver goals.
Determines treatment strategies and delivers Occupational Therapy services that promote health and wellness, prevent injury or disability, improve and develop self-dependency and maintenance, productivity, leisure, perceptual motor function, rest, mental, cognitive physical, sensory-perceptual, psychosocial developmental, and emotional wellbeing in a variety of contexts and environments in order to maximize function in the areas of daily living, leisure/recreation, and productivity/work.
Delegates patient care responsibilities to Certified Occupational Therapy Assistant.
Performs thorough evaluation of patient’s level of function upon Physician referral, including evaluations for upper extremity strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, perceptual-cognitive motor function, and functional abilities of daily living.
Recognizes physiological, psychological, and age related changes in the patient.
Consults with other Health Care Team members as necessary.
Develops an individualized patient-centered treatment plan of care and evaluates it on an ongoing basis, revising as necessary.
Establishes measurable goals with the patient/family based on appropriate patient care protocols and critical paths.
Notifies Physician of any changes in status or progress.
Delivers therapeutic techniques appropriate to patient’s needs and medical status.
Implements, delivers, and delegates patient care by utilizing appropriate interventions and clinical decision making.
Utilizes therapeutic techniques to support everyday activities, including fine motor skills, exercises for gross motor skills, and techniques to maximize function in daily living, leisure/recreation, and productivity/work.
Provides patient and caregiver education regarding treatment, precautions, and equipment use.
Completes all documentation including evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary.
Ensures timely and accurate documentation and coding of skilled and medically necessary services provided to the patient.
Participates in patient transition planning and/or patient discharge planning, including recommendations to community resources.
Coordinates patient care with other care providers such as Rehab Therapists, Nurses, Social Workers, and other community resources as related to care transition planning.
Delegates patient care tasks and provides clinical supervision for Occupational Therapist students, Rehabilitation Aides and Certified Occupational Therapy Assistants.
Maintains a clean, neat work environment for patients and staff, adhering to infection control and safety precautions.
Restocks supplies as necessary.
Maintains clinical proficiency, continuing education units (CEUs), annual health stream, and all education requirements as required by the State of Connecticut licensure.
Performs other related duties as required.
