Occupational Therapist
About The Position
VNA Health at Home, part of CommonSpirit Health at Home, is seeking a skilled and compassionate Occupational Therapist for its VNA Home Health team. The Occupational Therapist provides skilled therapy services to patients in their homes, evaluating their abilities and needs, setting goals, and developing plans to promote independence, improve quality of life, and prevent potential health-related problems. The role includes assessing patients’ physical, cognitive, and psychosocial abilities; implementing personalized treatment plans; educating patients and families; maintaining accurate records; monitoring progress; and collaborating with other members of the home health team. Requirements
- Bachelor’s degree from an accredited school approved by the American Occupational Therapy Association.
- Active registered occupational therapist license in the state of practice.
- Obtain and maintain current CPR/BLS certification.
- One year of Occupational Therapy experience in a clinical setting.
- Current driver’s license.
- Comply with federal, state, and local laws and regulations.
Responsibilities
- Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs.
- Develop and implement personalized treatment plans, which may include exercises, adaptive techniques, and assistive devices to improve patients’ functional abilities.
- Teach patients and family members about home safety, adaptive equipment, and strategies for managing daily activities.
- Maintain accurate and timely records of patient progress, treatment plans, and therapy program modifications in compliance with regulatory standards.
- Monitor patients’ progress and adjust treatment plans as their needs change.
- Use outcome-based measures to assess the effectiveness of therapy interventions.
- Collaborate and communicate patient condition changes with physicians, nurses, physical therapists, speech therapists, and other home health team members.
