Concept Rehab is seeking a dedicated Occupational Therapist (OT) to join their therapy team at a Skilled Nursing facility in Kettering and Centerville, Ohio. The facilities are located 3 miles apart. Concept Rehab is a therapist-owned company with over 40 years of experience providing rehabilitation services in various settings. They are committed to helping patients maximize independence, improve function, and achieve meaningful outcomes.
Requirements
- Bachelor's, Master's, or Doctorate Degree in Occupational Therapy
- Current Occupational Therapist license in the state of practice
- Strong communication and interpersonal skills
- Ability to work independently and within a collaborative healthcare team
- Commitment to patient-centered care and clinical excellence
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide skilled occupational therapy services
- Help patients improve daily living skills, mobility, cognition, strength, and functional independence
- Monitor progress and adjust treatment plans as needed
- Supervise Occupational Therapy Assistants (OTAs)
- Collaborate with physicians, nurses, families, and interdisciplinary team members
- Participate in care plan and discharge planning meetings
- Educate patients, caregivers, and facility staff
- Recommend equipment and interventions to improve patient outcomes
- Maintain timely, accurate documentation and billing
- Comply with all state licensure, regulatory, and professional practice standards
