About The Position

FOX Rehabilitation is expanding its team in Houston, TX, seeking passionate, licensed Occupational Therapists to provide exceptional one-on-one care to older adults. This role involves delivering therapy directly in patients' private residences or within Senior Living Communities, utilizing FOX's signature Geriatric House Calls™ model. The primary goal is to help patients improve mobility, independence, and overall quality of life. The position offers significant autonomy and flexibility, allowing therapists to create and manage their own treatment schedules and foster strong, lasting patient relationships. Clinicians also have the option to incorporate telehealth and other virtual care methods to enhance flexibility and ensure continuity of care. FOX Rehabilitation is a clinician-operated practice dedicated to meaningful relationships, evidence-based care, and combating ageism, providing a supportive environment led by experienced clinical professionals.

Requirements

Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
Degree from an accredited Occupational Therapy program
Basic computer and EMR proficiency
Current CPR certification
Ability to meet the physical demands of in-home and community-based care, including: Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
Performing CPR
Providing moderate to maximum assistance with patient transfers and bed mobility
Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
Wearing PPE when required

Responsibilities

Provide one-on-one Occupational therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
Document effectively within FOX’s EMR system in accordance with regulatory standards
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
Provide telehealth / virtual care when appropriate to extend access and flexibility
