Occupational Therapist - Full-Time / Part-Time or Per-Diem
About The Position
Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with others living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients. This role comprises 100% travel within our service region, traveling to patients’ homes. We are hiring for Full-Time 40 hours per week M-F 8am-5pm or Part-Time/Per-Diem role of between 10-20 hours per week, with the possibility of expanded hours in the future.
Requirements
- One year experience as a home health care provider
- Current California State license as an Occupational Therapist and stays updated through independent study and continuing education
- Current CPR certification
- Valid California Driver’s License
- Eligible to be insured under our liability policy with a clean DMV report
- Your own vehicle and proof of current auto insurance
- COVID vaccinated or have an exemption
Responsibilities
- Evaluate patients upon physician diagnosis
- Assess patient and identify deficits impacting self-care, functional mobility, behavior, cognition and patient’s ability to function in their life’s role
- Deliver occupational therapy to patients in their own homes
- Help patients improve their motor skills, coordination, and balance through therapeutic exercises and activities
- Assist patients in developing or regaining daily living skills, such as dressing, bathing, and eating
- Teach patients how to use adaptive equipment and assistive devices to enhance their independence
- Collaborate with other healthcare professionals, such as physicians, nurses, social workers, physical therapists and speech therapists, to coordinate patient care
- Monitor and document patients' progress during therapy sessions
