About The Position

VNA Health at Home, part of CommonSpirit Health, seeks a skilled and compassionate Occupational Therapist to provide therapy services to patients in their homes. The role evaluates patients’ abilities and needs, establishes goals, and develops plans to promote independence, improve quality of life, and prevent potential health-related problems. The Occupational Therapist works with patients and families and collaborates with physicians, nurses, physical therapists, speech therapists, and other home health team members.

Requirements

Bachelor’s degree from an accredited school approved by the American Occupational Therapy Association.
Active registered occupational therapist license in the state where the candidate practices.
Current CPR/BLS certification, which must be obtained and maintained.
One year of occupational therapy experience in a clinical setting.
Current driver’s license.
Comply with federal, state, and local laws and regulations.

Responsibilities

Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs.
Develop and implement personalized treatment plans, which may include exercises, adaptive techniques, and assistive devices to improve functional abilities.
Teach patients and family members about home safety, adaptive equipment, and strategies for managing daily activities.
Maintain accurate and timely records of patient progress, treatment plans, and therapy program modifications in accordance with compliance and regulatory standards.
Monitor patient progress and adjust treatment plans as needs change.
Use outcome-based measures to assess the effectiveness of therapy interventions.
Collaborate with physicians, nurses, physical therapists, speech therapists, and other home health team members, and communicate changes in patients’ conditions.
