About The Position

The Occupational Therapist evaluates patients and develops individualized treatment plans designed to improve functional independence and quality of life. Services are provided in home health settings and focus on helping patients safely perform activities of daily living (ADLs), improve upper extremity function, and adapt to physical limitations. Requirements

  • Master's Degree or Doctorate in Occupational Therapy from an accredited occupational therapy program
  • Current Occupational Therapist license in the state where services are provided
  • Successful completion of all state-required licensing requirements
  • Minimum of two (2) years of experience as an Occupational Therapist
  • Strong clinical assessment, communication, and documentation skills
  • Commitment to maintaining HIPAA compliance and patient confidentiality

  • Home health or community-based experience preferred

Responsibilities

  • Evaluate patients and develop individualized treatment plans based on physician orders and clinical findings
  • Assess patients' ability to perform activities of daily living, including dressing, bathing, grooming, meal preparation, and household tasks
  • Develop therapeutic interventions to improve functional mobility, coordination, strength, cognition, and independence
  • Recommend adaptive equipment, assistive devices, and environmental modifications to promote safety and independence
  • Educate patients, family members, and caregivers on therapeutic techniques, home safety, and proper use of adaptive equipment
  • Identify safety concerns and fall risks within the home environment and recommend appropriate interventions
  • Monitor patient progress and adjust treatment plans as needed to achieve established goals
  • Collaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate care
  • Communicate changes in patient condition and treatment response to the physician and Director of Nursing
  • Complete accurate clinical documentation, progress reports, and discharge summaries in a timely manner
  • Treat clients, families, caregivers, and team members with professionalism, courtesy, and respect
