About The Position

As the Occupational Therapist for our Program of All-Inclusive Care for the Elderly (PACE) in San Gabriel Valley, you will be a vital part of our interdisciplinary team at Seen Health. Your role will focus on assessing participants' functional abilities and designing customized interventions to promote independence, safety, and overall well-being.

Requirements

  • Graduate of an accredited occupational therapy program; Master's degree in Occupational Therapy (OT) required.
  • Valid and active California Occupational Therapy license.
  • Minimum of 2 year of experience in occupational therapy, with a focus on geriatric or community-based care preferred.
  • Strong clinical assessment and treatment planning skills.
  • Ability to work collaboratively in a multidisciplinary team environment and communicate effectively with participants, families, and colleagues.
  • Knowledge of adaptive equipment, home modification strategies, and community resources for older adults.

Nice To Haves

  • Proficiency in Mandarin and/or Cantonese preferred but not required.

Responsibilities

  • Conduct comprehensive occupational therapy assessments to evaluate participants’ cognitive, physical, and psychosocial functioning.
  • Develop individualized treatment plans based on assessment findings, participant goals, and input from the Interdisciplinary Team (IDT).
  • Provide direct occupational therapy interventions, including therapeutic activities, adaptive equipment recommendations, home safety assessments, and caregiver education.
  • Collaborate with participants, families, caregivers, and other healthcare professionals to establish realistic goals and promote active participation in therapy sessions.
  • Document participant progress, treatment plans, and outcomes accurately and in a timely manner using electronic medical records.
  • Monitor and reassess participant response to therapy interventions, modifying treatment plans as needed to optimize functional independence and quality of life.
  • Coordinate with external service providers, such as equipment vendors and community resources, to facilitate continuity of care and support participants' transition back to their homes.
  • Participate actively in IDT meetings, contributing insights and recommendations regarding participants’ occupational therapy needs and progress.
  • Adhere to center policies and procedures, maintain confidentiality of participant information, and comply with regulatory requirements.
