About The Position

This role is based in an Assisted Living, Independent Living, and Memory Care setting. The primary responsibility is to provide Occupational Therapy and related services for patients while maintaining positive interactions with communities and clients, adhering to Reliant Rehabilitation guidelines. Key duties include screening and evaluating patients, developing treatment and discharge plans, supervising assistants and rehab techs, and managing cases. The position requires maintaining professional conduct, adhering to state and federal regulations, and ensuring quality patient care.

Requirements

  • Graduate of an ACOTE accredited school of occupational therapy.
  • Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.).
  • Basic Internet navigation skills.
  • Valid certification from the National Board for Certification of Occupational Therapy (NBCOT).
  • Valid Occupational Therapist state license (where applicable).
  • Effective clinical problem-solving skills.
  • Be a strong team player.
  • Strong verbal & written communication skills.
  • Ability to build strong relationships with a diverse population.
  • Ability to read, write, speak, and comprehend in English: instructions, correspondence, charts, memos, and reports.
  • Ability to work in a fast-paced, productive work environment.
  • Must be able to travel between communities during scheduled hours.
  • Must be able to work varying hours and shifts, including weekends and holidays.
  • Must be able to lift fifty (50) pounds of dead weight alone.

  • Prior to beginning patient care for Reliant Rehabilitation, a peer OT may complete a Competency Checklist.

Responsibilities

  • Optimize the functionality and skills of patients by providing, directing, and/or overseeing occupational therapy, including screening and evaluation of residents.
  • Develop and implement initial and ongoing treatment plans of care and discharge plans.
  • Comply with state and federal guidelines regarding accurate and timely billing and documentation in the medical record.
  • Establish and maintain effective interactions with patients, the interdisciplinary team, and other clientele, reflecting patient-centered care.
  • Refer patients for treatment to Certified Occupational Therapy Assistants (COTA) and provide consultation and clinical supervision to COTAs.
  • Directly supervise COTAs and Rehab Techs in skilled tasks during group and individual treatment.
  • Actively integrate occupational therapy’s role into community and interdisciplinary treatment team activities.
  • Contribute to community patient care, utilization review, case management, administrative staff/department head, and family conference activities.
  • Establish and maintain a good working relationship with other healthcare professionals and colleagues in the community.
  • Make appropriate referrals to other members of the rehabilitation team.
  • Develop and implement new programs/techniques to enhance rehabilitation services.
  • Adhere to Reliant Rehabilitation and client-specific policies and procedures related to environmental safety, orderliness, risk management, infection control, and emergency preparedness.
  • Adhere to the requirements of patient’s rights and privacy.
  • Enhance clinical expertise, professional and management skills through interaction with managers, therapists, other professional staff, self-study, and continuing education.
  • Maintain awareness of issues related to the profession of occupational therapy and the healthcare environment.
  • Understand various relevant payment models related to billing and treatment guidelines, and clinically appropriate means/modes of delivery.
  • Provide quality care with reasonable goals and outcomes within guidelines.
  • Be compliant with infection control procedures and environmental safety protocol within a community.
  • Perform patient lifting using appropriate biomechanical techniques, capable of lifting fifty (50) pounds of dead weight alone.
  • Maintain confidentiality of Reliant Rehabilitation, community, and patient information.
  • Conduct self in an ethical, legal, and responsible manner at all times.
  • Adhere to the policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct.
  • Travel between communities during scheduled hours.
  • Work varying hours and shifts, including weekends and holidays.
  • Attend all mandatory meetings, trainings, and assignments.
  • Perform other duties and responsibilities as assigned.
