About The Position
Ultimate Therapy is hiring a full-time Occupational Therapist to provide hands-on rehab services in a skilled nursing facility. This position focuses on helping residents improve independence with daily tasks and overall function.
Requirements
- Active Occupational Therapist license required

- Skilled nursing experience preferred
Responsibilities
- Evaluate residents and establish individualized care plans
- Deliver treatment targeting self-care, coordination, cognition, and daily routines
- Introduce adaptive methods and assistive tools to support independence
- Monitor outcomes and adjust interventions as needed
- Collaborate with clinical team members and communicate with families
- Complete accurate and timely documentation
