PRN Occupational Therapist

Requirements
- Graduate degree in Occupational Therapy from an accredited university
- Valid Occupational Therapy license in the state of Iowa
- Current CPR certification
- Minimum 1 year of experience in home care, skilled nursing facility, acute care, or inpatient rehab
- Valid driver’s license
- Access to a reliable vehicle and active car insurance
Nice To Haves
- 1 year of home health experience preferred
Responsibilities
- Conduct home occupational therapy visits including evaluations, follow-up visits, 30-day re-evaluations, recertifications, and discharges
- Evaluate patient rehabilitative status and contribute to the development of the plan of care
- Initiate occupational therapy programs and guide other personnel involved in patient care
- Communicate with physicians regarding therapy frequency, duration, and goals
- Prepare and submit clinical documentation and progress summaries in a timely manner
- Educate patients and caregivers on treatment plans and home exercise programs
- Assist patients in achieving functional goals through therapeutic exercises, transfer training, neuromuscular re-education, and coordination activities
- Provide education on safety and fall prevention to improve participation in activities of daily living
- Assess vital signs including blood pressure, heart rate, and oxygen saturation as needed and report abnormalities
- Educate patients on the proper use of durable medical equipment (DME) and recommend equipment as appropriate
- Document patient progress and adjust treatment plans based on clinical findings
- Coordinate discharge planning with physicians, nurses, social workers, and other care team members
- Collaborate effectively with interdisciplinary team members including SN, OT/COTA, PT/PTA, SLP, and physicians
- Maintain patient confidentiality and comply with all organizational policies and procedures
- Follow infection control and safety protocols at all times
- Maintain professional knowledge through continuing education and professional development
- Perform other duties as assigned
