Requirements

  • Graduate degree in Occupational Therapy from an accredited university
  • Valid Occupational Therapy license in the state of Iowa
  • Current CPR certification
  • Minimum 1 year of experience in home care, skilled nursing facility, acute care, or inpatient rehab
  • Valid driver’s license
  • Access to a reliable vehicle and active car insurance

Nice To Haves

  • 1 year of home health experience preferred

Responsibilities

  • Conduct home occupational therapy visits including evaluations, follow-up visits, 30-day re-evaluations, recertifications, and discharges
  • Evaluate patient rehabilitative status and contribute to the development of the plan of care
  • Initiate occupational therapy programs and guide other personnel involved in patient care
  • Communicate with physicians regarding therapy frequency, duration, and goals
  • Prepare and submit clinical documentation and progress summaries in a timely manner
  • Educate patients and caregivers on treatment plans and home exercise programs
  • Assist patients in achieving functional goals through therapeutic exercises, transfer training, neuromuscular re-education, and coordination activities
  • Provide education on safety and fall prevention to improve participation in activities of daily living
  • Assess vital signs including blood pressure, heart rate, and oxygen saturation as needed and report abnormalities
  • Educate patients on the proper use of durable medical equipment (DME) and recommend equipment as appropriate
  • Document patient progress and adjust treatment plans based on clinical findings
  • Coordinate discharge planning with physicians, nurses, social workers, and other care team members
  • Collaborate effectively with interdisciplinary team members including SN, OT/COTA, PT/PTA, SLP, and physicians
  • Maintain patient confidentiality and comply with all organizational policies and procedures
  • Follow infection control and safety protocols at all times
  • Maintain professional knowledge through continuing education and professional development
  • Perform other duties as assigned
