About The Position

Occupational Therapist (OT) – Home Health Ocean County, NJ | Full-Time Be the Occupational Therapist you always wanted to be—at BAYADA Home Health Care. BAYADA Home Health Care is seeking a compassionate and dedicated Occupational Therapist (OT) to join our Ocean County Visits team. You'll have the opportunity to provide meaningful, one-on-one care that helps adult and geriatric clients remain safe, independent, and comfortable in their own homes throughout Ocean County, NJ. Requirements

  • Current New Jersey Occupational Therapist (OT) license.
  • Graduate of an accredited Occupational Therapy program.
  • Minimum of one year of recent OT experience.
  • Strong clinical, organizational, communication, and problem-solving skills.
  • Ability to work independently and effectively manage time.

Nice To Haves

  • Prior home health experience preferred but not required.
  • Experience with EMR documentation preferred.

Responsibilities

  • Providing skilled home visits within designated territories.
  • Performing comprehensive evaluations of functional abilities, including ADLs, IADLs, mobility, cognition, communication, and home safety.
  • Developing individualized treatment plans to improve functional outcomes and independence.
  • Assessing home environments and recommending adaptive equipment and modifications.
  • Collaborating with nurses, therapists, physicians, and other healthcare professionals to deliver patient-centered care.
  • Educating patients, families, and caregivers on therapeutic interventions and safety strategies.
  • Accurately documenting assessments, treatments, and patient progress using electronic medical records.
  • Participating in interdisciplinary care planning to achieve the best possible outcomes for patients.
