We are seeking a compassionate and skilled Occupational Therapist to serve patients in the Ft. Myers, FL area. In this role, you will partner closely with patients throughout their plan of care, focusing on restoring function, improving mobility, and ensuring safety within the home environment. Your work will directly enhance quality of life through prevention, treatment, and rehabilitation, all under a physician-directed plan of care and in collaboration with patients and caregivers.

Requirements

Current Occupational Therapist license in the state of Florida
Strong clinical knowledge and decision-making skills
Ability to work independently and manage diverse patient needs
Excellent communication and interpersonal skills
Commitment to ongoing professional development
This position requires background screening through the Florida Care Provider Background Screening Clearinghouse.

Nice To Haves

Home health experience preferred (minimum of 2 years)

Responsibilities

Follow and uphold established clinical policies and procedures
Evaluate patients and develop individualized treatment plans
Perform skilled therapeutic interventions as ordered by the physician
Promote independence and improve patients’ functional abilities
Collaborate with physicians and interdisciplinary care teams
Educate patients and caregivers on home programs and daily living activities
Participate in care planning and patient case conferences
Complete timely and accurate clinical documentation, including OASIS assessments
Modify care plans as patient conditions evolve
