About The Position

Moraga Post Acute is seeking a dedicated and compassionate Occupational Therapist (OTR) to join our rehabilitation team on a PRN basis. Requirements

  • Current California Occupational Therapist (OTR) License required.
  • Strong communication, organizational, and clinical skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Commitment to delivering compassionate, resident-centered care.

Responsibilities

  • Evaluate residents and develop individualized treatment plans based on their functional needs and rehabilitation goals.
  • Provide skilled occupational therapy services to improve independence with activities of daily living (ADLs).
  • Supervise and collaborate with Occupational Therapy Assistants (COTAs) in accordance with state regulations.
  • Communicate effectively with physicians, nursing staff, residents, families, and the interdisciplinary care team.
  • Participate in care conferences, rehabilitation meetings, and discharge planning.
  • Educate residents and caregivers on treatment goals, home programs, safety techniques, and fall prevention.
  • Recommend adaptive equipment and durable medical equipment (DME) to support resident independence.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Contribute to quality improvement initiatives and staff education programs.
