About The Position

Stern at Home Therapy is hiring Occupational Therapists to provide one-on-one home health care to a primarily geriatric patient population throughout Rockland County, New York. The role offers part-time and full-time options, with W-2 salary, W-2 visit-based, and 1099 contractor compensation models. Full-time 1099 visit-based clinicians can earn up to $145,000 annually, depending on visit volume. Therapists manage their availability and caseload while providing individualized care without double-booking or productivity quotas. Home health experience is preferred but not required.

Requirements

Active Occupational Therapist license in New York.
Reliable transportation.
Strong clinical, communication, and time-management skills.
Ability to work independently in a home-based setting.

Responsibilities

Perform occupational therapy evaluations and develop individualized plans of care.
Provide one-on-one treatments based on each patient's functional needs and goals.
Manage individualized treatment plans with clinical autonomy.
Help patients improve mobility, strength, balance, safety, and overall function.
Complete accurate and timely clinical documentation.
Communicate with patients, families, physicians, and members of the clinical team.
Coordinate with the administrative team regarding scheduling, patient care, and logistics.
Manage caseload based on availability and employment structure.
