About The Position
FOX Rehabilitation is seeking passionate, licensed Occupational Therapists to deliver exceptional one-on-one care to older adults in West Hartford, Bloomfield, and Avon, CT. The role involves treating patients in private residences and Senior Living Communities, helping them improve mobility, independence, and quality of life through the Geriatric House Calls™ model. This opportunity is ideal for OTs who value autonomy, flexibility, and meaningful patient outcomes, with the freedom to manage their own schedules. Clinicians may also incorporate telehealth and virtual care, offering greater flexibility and improved continuity of care. This is a high-impact OT role focused on positively impacting the lives of the communities served.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
Responsibilities
- Provide one-on-one Occupational therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Document effectively within FOX’s EMR system in accordance with regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
