This is a field-based opportunity for an Occupational Therapist (OTR/L) who values autonomy, efficiency, and meaningful one-on-one care. Requirements

• Active Minnesota OTR/L license.
• Strong self-management and ability to work independently.
• Organized, efficient, and outcome-driven mindset.
• Reliable transportation for field-based visits.
• Passion for helping patients regain independence in their home.

Responsibilities

• In-home OT evaluations focused on safety, function, and independence.
• Treatment plans that drive measurable ADL and mobility outcomes.
• Hands-on therapy to improve strength, coordination, and daily function.
• Home safety assessments and adaptive equipment recommendations.
• Efficient territory management to maximize your time and impact.
• Accurate EMR documentation and collaboration with the care team.
