About The Position

Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, is seeking PRN Occupational Therapists (OT) to join their award-winning team. The role involves delivering compassionate healthcare in patients' homes, focusing on assessing functional status, evaluating home environments, developing care plans, and retraining patients in daily activities. The service area covers Melbourne to Palm Bay/Micco/Sebastian areas, including adjacent beachside locations.

Requirements

  • Must have a current OT license (state-specific).
  • Graduate of an accredited occupational therapy program recognized by the American Medical Association and the American Occupational Therapy Association.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Valid BLS certification.
  • Minimum two years of OT experience (home health experience preferred).
  • Excellent communication, organizational, and time management skills.
  • Must have a valid driver’s license and access to a reliable and insured vehicle.
  • Ability to visit homes that may not be wheelchair accessible.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
  • Engage in physical activities such as walking, standing, stooping and occasional lifting or turning of patients.

Responsibilities

  • Assess the patient’s functional status (muscle function, endurance, visual coordination, communication skills, self-care, work capacity, etc.).
  • Evaluate the home environment for hazards or barriers to independent living and identify necessary equipment.
  • Collaborate in developing a comprehensive care plan.
  • Design a treatment program with goals for improved function and communicate the plan to the referring physician and other team members.
  • Teach or retrain patients in daily activities lost due to illness or injury, following agency policies.
  • Fabricate splints and instruct patients on using adaptive equipment to enhance function.
  • Provide training in the use of prosthetic and/or orthotic devices.
  • Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
  • Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
  • Work alongside physicians and discharge planners to provide health education to patients and families/caregivers.
  • Identify patient and family/caregiver needs for additional services and make appropriate referrals.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Supervise Certified Occupational Therapy Assistants in accordance with agency policies and state regulations.
