About The Position

Innovate Rehab and Wellness is seeking a Licensed Occupational Therapist to provide patient-centered care in a skilled nursing facility at Onyx Care of Winnfield in Winnfield, Louisiana. The therapist evaluates patients’ functional abilities and therapy needs, develops and delivers individualized treatment plans, and helps patients improve motor skills, coordination, and daily function so they can regain independence and quality of life. The role emphasizes one-on-one therapy; group and concurrent treatments make up less than 1% of services. Schedules are described as realistic and flexible to support work-life balance. Innovate Rehab and Wellness is a therapist- and veteran-owned provider of skilled health care services with more than 25 years of experience. Requirements

Master’s or doctorate in Occupational Therapy from an accredited program.
Valid state occupational therapy license.
Strong communication and problem-solving skills.
Passion for patient-centered care.

Nice To Haves

Experience as an occupational therapist is preferred; new graduates are welcome.
CPR certification preferred.

Responsibilities

Conduct thorough occupational therapy evaluations.
Develop and adjust personalized treatment plans.
Use therapeutic techniques and adaptive equipment.
Train patients and caregivers on exercises and assistive devices.
Educate patients on strategies to improve function and safety.
Collaborate with physical therapists, speech-language pathologists, physicians, and the full care team.
Document patient progress and treatment changes accurately.
Stay current with occupational therapy best practices and research.
