At Innovive, we believe that when our clinicians thrive, our patients receive the best care. We are seeking a compassionate and skilled Occupational Therapist (OT) to join our Home Health team. In this role, you will provide skilled occupational therapy services to patients in their homes, helping them improve independence, safety, and quality of life. Working collaboratively with physicians, nurses, rehabilitation professionals, and caregivers, you will evaluate patient needs, develop individualized treatment plans, and deliver evidence-based interventions that support successful outcomes. This position requires strong clinical judgment, excellent communication skills, and a commitment to providing high-quality, patient-centered care in a home health setting.

Requirements

  • Graduate degree in Occupational Therapy from an accredited program.
  • Current and unrestricted Massachusetts Occupational Therapist license.
  • Current CPR certification.
  • Minimum of 1 year of experience in home health, skilled nursing, acute care, or inpatient rehabilitation.
  • Valid driver's license, reliable transportation, and active automobile insurance.
  • Ability to travel throughout assigned service area to conduct patient visits.

Responsibilities

  • Conduct home-based occupational therapy evaluations, follow-up visits, 30-day reassessments, recertifications, and discharge visits.
  • Assess patients' functional abilities, rehabilitation potential, and occupational therapy needs.
  • Develop, implement, and modify individualized treatment plans in collaboration with the interdisciplinary care team.
  • Establish therapy goals and communicate treatment recommendations, visit frequency, and progress updates to physicians and other healthcare providers.
  • Provide therapeutic interventions, including: Activities of daily living (ADL) training, Transfer and mobility training, Neuromuscular re-education, Therapeutic exercise, Coordination and balance training, Home safety and fall prevention education.
  • Educate patients, families, and caregivers on treatment plans, home exercise programs, adaptive techniques, and equipment use.
  • Assess and monitor patient vital signs as appropriate and report significant findings to the Rehabilitation Manager and primary care provider.
  • Recommend, educate, and coordinate durable medical equipment (DME) needs, including walkers, wheelchairs, grab bars, shower chairs, canes, crutches, and prosthetic devices.
  • Maintain timely, accurate, and compliant clinical documentation within the electronic medical record (EMR) system.
  • Collaborate with physicians, nurses, social workers, PTs, PTAs, SLPs, COTAs, and other healthcare professionals to support patient care and discharge planning.
  • Promote continuity of care through patient and caregiver education, discharge planning, and referral recommendations.
  • Adhere to all agency policies, infection control procedures, regulatory requirements, and professional standards.
  • Participate in ongoing professional development and continuing education activities.
  • Perform other duties as assigned.
