Occupational Therapist - Home Health Senior Living
About The Position
BAYADA Home Health Care is seeking an Occupational Therapist to provide home health visits in senior living communities throughout Franklin and Adams counties. The Occupational Therapist is an integral member of a multidisciplinary health care team providing rehabilitative care and skilled nursing to clients so they can receive the medical care they need while remaining at home. The role involves evaluating patients’ functional, cognitive, psychological, and motor performance; creating and revising individualized intervention and care plans; assessing home environments and equipment needs; and educating patients and their representatives. The position also requires accurate documentation of care using a touch pad tablet. Full-time, part-time, and per-diem opportunities are described. Base pay is $58.00–$68.00 per point, depending on qualifications and subject to negotiation.
Requirements
- Current Pennsylvania Occupational Therapist license.
- At least one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, documented by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills.
Nice To Haves
- Prior home care experience.
- Prior experience with electronic medical records (EMR).
Responsibilities
- Make home visits to clients in designated geographic territories.
- Evaluate patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL and IADL performance.
- Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans.
- Evaluate home environments for hazards or barriers to independent living.
- Identify equipment needs and environmental adaptations to improve client outcomes.
- Continually assess and revise occupational therapy care plans and participate in multidisciplinary care planning with nursing, as appropriate.
- Educate and instruct patients, family members, or other representatives in rehabilitative care and activities that promote health, safety, and independent living.
- Document observations, interventions, evaluations, and services provided using a touch pad tablet.
