About The Position

Trillium Health Resources is seeking a Complex Transitional Care Occupational Therapist (OT) to join their team. This role involves leading occupational therapy services by overseeing staff, clinical operations, and service delivery. The Occupational Therapist will provide clinical and operational oversight, conduct clinical assessments, make recommendations for care planning, support staff development, and ensure services meet practice standards. The position aims to support effective rehabilitation services and promote positive functional outcomes. The work impacts individuals with serious mental health needs, substance use disorders, traumatic brain injuries, and intellectual/developmental disabilities across 46 counties in North Carolina.

Requirements

  • Master’s degree in Occupational Therapy
  • One (1) year of experience working with the co-occurring of physical health and behavioral health, physical health, or chronic condition population.
  • Fully licensed by the North Carolina Board of Occupational Therapy as an Occupational Therapist.
  • Must have a valid driver’s license.
  • Must reside within Trillium’s Mid State Region, which includes the following counties: Anson, Guilford, Montgomery, Randolph, and Richmond.
  • Must be able to travel within catchment as required.

Responsibilities

  • Leads occupational therapy services and staff.
  • Provides clinical supervision and guidance to occupational therapy staff and support personnel.
  • Conducts clinical assessments and provides recommendations to support care planning and member needs.
  • Monitors therapy outcomes and functional progress to support effective service delivery.
  • Ensures compliance with clinical standards, regulatory requirements, and documentation practices.
