About The Position

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time opportunity performing Senior Living visits. This role involves servicing adult and geriatric patients on a per-visit basis in designated senior living communities or group homes. As a home care Occupational Therapist, you will be a vital part of a multi-disciplinary team providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.

Requirements

A current New Hampshire Occupational Therapist license.
A minimum of one year of recent work experience as an Occupational Therapist.
Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal skills, organizational skills, and problem-solving skills.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Nice To Haves

Prior home care experience preferred, but not required.

Responsibilities

Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group homes for adults with intellectual disabilities.
Utilize current research-based approaches to care to maximize functional independence for clients, operating professionally at the top of licensure.
Collaborate with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment.
Educate clients, families, and community caregivers on created care plans to foster a team approach for maximizing functional independence.
Perform evaluations, treatments, and discharge planning for clients.
Utilize the outpatient EMR platform to deliver and document individualized treatment approaches to meet client needs.
