The Occupational Therapist (OT) is responsible for the daily management of an assigned patient caseload in home health. This role ensures timely care and documentation to meet patient needs as prescribed in the plan of care and per physician orders. The OT collects pertinent data from patient assessments, telephonic contacts, and EMR information. They evaluate functional abilities, develop plans of care in conjunction with the physician, and perform certified OT services. This position directs care coordination, supervises COTAs and HHAs, and designs/fits orthotic devices and adaptive equipment. The OT instructs family members and staff in therapy methods, observes and reports patient condition changes to the physician, and maintains communication with the multidisciplinary team. They incorporate short and long-term goals into therapy, support effective communication with physician oversight and branch supervision, and provide clear, timely, and complete documentation. The OT adheres to productivity standards and performs ordered treatments in accordance with state and federal regulations, JCAHO, Medicare Guidelines, OASIS, and agency policies. They delegate elements of care with minimal supervision and promote shared governance and research to improve clinical outcomes. The OT attains and integrates current occupational therapy practice, evidence, and research findings, contributing to the quality of practice and communicating effectively in all areas. They demonstrate leadership, collaborate with patients and families, and evaluate their own practice against professional standards. The OT utilizes appropriate resources for safe, effective, and financially responsible services.
