About The Position

As an Occupational Therapist, you’ll deliver skilled, one‑on‑one therapeutic interventions directly in patients’ homes, helping them regain independence after surgery, injury, or chronic illness. You will assess functional abilities within each patient’s unique home environment and create personalized treatment plans focused on maximizing success with activities of daily living (ADLs). Requirements

  • Bachelor's degree in occupational therapy.
  • Occupational Therapist License - NM Regulation & Licensing Board and BLS required at time of hire.
  • Requires detailed knowledge of occupational therapy in terms of what and how work is to be done as well as why it is done, this level includes interpretation of data.
  • Basic computer skills required.

Responsibilities

  • Evaluates and treats patients using a broad range of therapeutic interventions to support rehabilitation, functional independence, and developmental progress.
  • Demonstrates a strong understanding of growth and development principles across the lifespan for all patient populations served.
  • Oversees and delivers a comprehensive rehabilitation plan tailored to the patient’s physical, cognitive, psychosocial, and environmental needs.
  • Promotes patient and caregiver self-determination by identifying and coordinating external resources that support the highest possible level of independence.
  • Collaborates and communicates effectively with interdisciplinary team members and services involved in patient care.
  • Aligns treatment strategies with discharge goals established during the initial evaluation and throughout the episode of care.
  • Serves as a dedicated patient advocate, ensuring patient needs and preferences remain central to care planning.
  • Performs additional duties as assigned.
  • Provides services across a variety of settings, which may include inpatient, outpatient, long-term care, community-based programs, and home health environments.
