About The Position

A premier, therapist-led national healthcare organization is seeking a Full-Time Occupational Therapist to join its growing Home Health division in the East Valley of Phoenix. This organization offers a unique structure: full-time benefits are unlocked at just 30 hours per week, with absolutely no weekend requirements. The role involves providing patient-centered care designed to improve independence and quality of life directly in patients' homes. Clinicians will have the autonomy of managing their own caseload, backed by an experienced interdisciplinary team that values expertise and clinical input.

Requirements

Degree in Occupational Therapy from an accredited institution.
Valid Arizona state licensure as an Occupational Therapist (or license-eligible).
Strong clinical assessment, diagnostic, and treatment planning skills.
Excellent communication and interpersonal skills with a patient-centered approach.
Ability to lift and assist patients as needed.
Ability to stand for extended periods and perform physical tasks related to home therapy.

Responsibilities

Evaluate clients' physical, emotional, and cognitive abilities using clinical observations and standardized tools.
Create personalized treatment plans to enhance functional abilities.
Educate clients and their families on modifying daily activities, including the use of adaptive equipment and home modifications.
Maintain accurate, timely records of evaluations, progress, and treatment plans in compliance with legal and ethical standards.
Advocate for clients' needs and support them in accessing community resources, services, and necessary accommodations.
