About The Position

Presbyterian Homes & Services is hiring an on-call Occupational Therapist at Johanna Shores in Arden Hills, Minnesota. The role serves older adults across multiple care settings, which may include transitional care/skilled nursing, assisted living, independent living, home health, and outpatient services. The therapist assesses residents and clients, develops and delivers treatment plans, supports progress toward quality-of-life goals, educates residents and their support networks, coordinates discharges, and maintains timely documentation and billing. The schedule offers weekday and weekend opportunities. The position is part of an in-house therapy team within a supportive organization that offers opportunities to learn and grow.

Requirements

Graduate from an accredited Occupational Therapy program.
Hold current licensure in the state or states of practice, as required.
Demonstrate compatibility with PHS's mission and operating philosophies.
Demonstrate the ability to read, write, speak, and understand English to communicate with customers.

Responsibilities

Conduct comprehensive assessments and evaluations using subjective and objective measures.
Develop treatment plans and programs in accordance with physician orders.
Provide quality treatments and actively progress goals to support the highest quality of life.
Conduct screens and provide education to residents or clients, families, caregivers, and staff.
Coordinate seamless discharges using the full continuum of therapy and fitness services.
Provide accurate and timely documentation and billing in the electronic medical record.
Demonstrate understanding of payer sources and regulatory requirements.
Complete and document supervisory visits according to regulations.
Communicate effectively with residents or clients, families, caregivers, physicians, referral sources, and the interdisciplinary team.
