About The Position

Banner Health is seeking an Occupational Therapist to join their Acute Care, Rehab team in Gilbert, Arizona. This role involves working in various hospital units including Med-Surg, Oncology, Stem-cell, ICU, PCU, ED, and Observation. The therapist will be an active change agent, dedicated to patient and family needs, and committed to safe, kind, effective, efficient, evidenced-based, and high-quality clinical care. The position offers support from a collaborative team and experienced leadership. The role requires a 4x10 hour shift schedule per week, with weekend and holiday coverage. Requirements

Must have successfully completed the academic requirements of an educational program in occupational therapy approved by the appropriate governing board.
Must possess a current Occupational Therapy license from the State of practice.
Current license from state of practice.
BLS certification required.
Ability to become proficient with computer software applications commonly used in an automated patient care environment.
Employees working for Banner Home Care AZ must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.

Nice To Haves

Oncology and acute care hospital experience is preferred.
Additional related education and/or experience preferred.

Responsibilities

Evaluates patient's functional abilities and limitations, determining if intervention is needed.
Establishes a plan of care and treatment goals, completing comprehensive evaluations within established timeframes.
Identifies and prioritizes key limiting factors and establishes functional, measurable, patient-related treatment goals.
Collaborates with patient and family/caregiver when setting goals.
Initiates discharge planning and recommends additions to or modifications of referring orders.
Implements individualized treatment plans considering the patient's physical, social, spiritual, cultural, educational, and age-specific needs.
Provides skilled treatment in a safe manner and assesses ongoing effectiveness of treatment, modifying the plan of care as indicated.
Involves patient and family/caregiver in the treatment and decision-making process.
Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.
Documents according to standards set by the department and regulatory agencies, completing documentation within established time frames.
Documents innovative and individualized treatment techniques/plans, reassessing treatment goals according to department policies.
Communicates pertinent patient information to ensure the best possible follow-through of care, including evaluation/treatment results and patient progress to physicians, nurses, patient, family/caregiver, co-workers, and the interdisciplinary team.
Communicates in a clear and timely manner and accepts responsibility to contact physicians for routine or difficult problem-solving.
Exhibits adaptability in assignments related to caseload, scheduling, and staffing shifts.
Takes initiative to assist in other patient care areas and accepts responsibility for all patient care needs.
Demonstrates appropriate direction and delegation of personnel (COTA, Therapy Technician, student) in compliance with department policies and regulatory agencies.
Participates in the training and orientation of new personnel and assures competency of personnel prior to delegation of tasks.
Participates in department/facility process improvements and meets productivity standards.
Interacts with all levels of staff, physicians, patients, families, and external contacts concerning patient healthcare.
Interacts with physician offices to report, require, or clarify information.
