About The Position

Activa Home Health (an AccordCare Company) is a licensed Medicare certified home health agency seeking an experienced per diem Field Occupational Therapist in Sebastian & Barefoot Bay. Requirements

  • Currently licensed as an Occupational Therapist (OT) in this state and in good standing with the Board in which he/she will practice.
  • Knowledge of principles, concepts and methodology of occupational therapy.
  • Ability to work well with an interdisciplinary team.
  • Ability to develop and maintain rapport with the Patient and family.
  • Excellent written and verbal communication skills.
  • Demonstrate problem solving and decision-making abilities.

Nice To Haves

  • At least one year or more work experience in occupational therapy in a multidisciplinary setting is preferred.

Responsibilities

  • Participates in the development of patient occupational therapy plans of care.
  • Makes visits to the patients for assessments and evaluations and communicates with the physician as needed.
  • Implements the Plan of Care.
  • Documents the care and submits electronic documentation in an accurate and timely manner to update and maintain the patient medical records.
  • Provides education and instruction to team members, patients and their families.
  • Participates in regularly scheduled interdisciplinary team meetings to coordinate the care of the patient and agency and family, exchange information and problem solve, and receive staff support and education.
  • Applies the policies and procedures of the agency and the rules and regulations of Federal and State Regulatory agencies and Accreditation Commission for Health Care in providing care.
  • Coordinates and develops patient/family plan of care and ensures the care plan is current, up to date and reviewed at every team meeting.
  • Assesses & evaluates patient's level of care, needs and requirements.
  • Completes electronic paperwork in an accurate and complete manner and adheres to agency policy for timely submission.
