About The Position

Human Touch Home Health is seeking a compassionate and skilled Occupational Therapist (OT) to join our growing team serving patients throughout Washington, DC. This is a PRN opportunity that offers flexibility, professional independence, and the ability to make a meaningful impact every day. As a Home Health Occupational Therapist, you'll help patients regain the skills, strength, and confidence they need to safely perform everyday activities while remaining in the comfort of their own homes. Requirements

Active Washington, DC Occupational Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Communication, and Documentation Skills

Home Health Experience Preferred but Not Required

Responsibilities

Complete occupational therapy evaluations, reassessments, and discharge visits
Develop individualized treatment plans based on patient goals and functional needs
Provide skilled occupational therapy services in patients' homes
Assess home environments and recommend adaptive equipment and safety modifications
Educate patients and caregivers on therapeutic techniques, home safety, energy conservation, and activities of daily living
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
