About The Position

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time or per diem opportunity. This role involves performing home health visits for adult and geriatric patients in senior living communities across Somerset, Middlesex, and Monmouth Counties. As a home care Occupational Therapist, you will be a vital part of a multi-disciplinary healthcare team, providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining in their homes.

Requirements

A current NJ Occupational Therapist license.
A minimum of one year of recent work experience as an Occupational Therapist.
Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal skills, organizational skills, and problem-solving skills.
Solid computer skills.

Nice To Haves

Prior home care experience preferred.
Prior experience with electronic medical records (EMR) preferred.

Responsibilities

Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group homes for adults with intellectual disabilities.
Operate professionally at the top of licensure, utilizing current research-based approaches to maximize clients' functional independence.
Collaborate closely with home health therapy partners to ensure appropriate transitions from home health to outpatient services, achieving client-centered goals within their living environment.
Educate clients, families, and community caregivers on care plans to foster a team approach for maximizing functional independence.
Conduct evaluations, treatments, and proper discharge planning for clients, utilizing the outpatient EMR platform to deliver and document individualized treatment approaches.
