About The Position

Our mission is to help seniors live independently, confidently, and joyfully through compassionate in-home care, personalized therapy, and innovative brain health programs. We are seeking an Occupational Therapist to provide in-home care for Medicare Part B patients. This role involves helping patients improve daily living skills, regain independence, and enhance their quality of life, all from the comfort of their homes.

Requirements

• Active Occupational Therapy (OT) license in the state of practice.
• Knowledge of Medicare Part B documentation and regulations.
• Strong assessment, treatment planning, and patient education skills.
• Excellent communication and interpersonal abilities.
• Reliable transportation and a valid driver's license.
• Comfortable working independently and managing a flexible, home-based schedule.
• Proficiency with electronic medical records (EMR) and basic computer skills.
• CPR and other relevant certifications as required.
• This position requires Level 2 background screening through Florida's Care Provider Background Screening Clearinghouse.

Nice To Haves

• Experience in home health or outpatient therapy preferred.

Responsibilities

• Assess patients in their homes and create personalized therapy plans.
• Provide skilled interventions to improve daily living skills and independence.
• Educate patients and caregivers on safety, exercises, and adaptive strategies.
• Document progress and ensure compliance with Medicare Part B regulations.
• Collaborate with the home health/ therapy team to coordinate patient care.
• Adjust treatments based on patient progress and needs.
