About The Position
Concept Rehab is a therapist-owned and operated company with over 45 years of experience providing high-quality care in Post-Acute and Skilled Nursing Facility (SNF) settings. They offer a supportive, growth-focused environment for clinicians. This role involves evaluating patients, developing individualized treatment plans, and providing skilled occupational therapy services to maximize patient independence and function. Responsibilities include addressing ADLs, mobility, safety awareness, and adaptive equipment needs, collaborating with various stakeholders, maintaining documentation, and educating patients and caregivers.
Requirements
- Degree from an accredited Occupational Therapy program
- Active state Occupational Therapist license
- Strong communication, clinical reasoning, and patient-centered care skills

Responsibilities
- Evaluate patients and develop individualized treatment plans
