About The Position

A division of Southwoods Health, Southwoods Health at Home is hiring a Per Diem Occupational Therapist. Will perform patient-centered occupational therapy evaluations and treatments that meet company, federal, state, and local standards, guidelines and regulations.

Requirements

Graduate of an accredited school for Occupational Therapy
Active Ohio OT licensure
Current AHA BLS certification
Strong knowledge of managing patients with upper extremity weakness, ADL deficits
Excellent communication and interpersonal skills
Ability to work effectively as part of a multidisciplinary team

Responsibilities

Administers medically prescribed occupational therapy treatment to patients suffering from injuries or disease to restore function, relieve pain and prevent disability.
Reviews physician referral/orders, patient’s condition and medical records to determine occupational therapy treatment needs.
Develops and revises occupational therapy treatment plans as appropriate based on evaluation and continued monitoring of patient status and response to treatment
Confers with physician and other practitioners to obtain additional patient information, suggest revisions in treatment program and integrate occupational therapy treatment with other aspects of patient’s health care.
Determines referral needs and makes applicable referrals in a timely manner.
Provides appropriate level of supervision for staff occupational therapist assistants, students, other licensed personnel and unlicensed personnel according to state and federal requirements.
Documents patient care services in accordance with regulatory requirements and within time expectation set by agency standards.
Provides education to patients and patient families as appropriate, including, but not limited to, information on post-op recovery processes, physician protocols, treatment plan, and home exercise programs.
May plan and conduct in-services or training programs on occupational therapy and related topics for medical staff, students and/or community groups.
Demonstrates flexibility in work schedule to meet patient scheduling needs.
