About The Position
The Occupational Therapist (OTR/L) – Home Health role focuses on helping patients achieve meaningful goals in their own homes, enabling them to overcome barriers to independent living. This position involves creating individualized treatment plans based on each patient's real-life environment, building strong relationships with patients and caregivers, and managing one's own schedule while collaborating with an experienced interdisciplinary team. The role emphasizes quality care over productivity quotas, allowing therapists to focus on transforming everyday living for patients by helping them regain independence in activities such as dressing, meal preparation, and showering.
Requirements
- Active Minnesota Occupational Therapist (OTR/L) license.
- Passion for helping patients remain independent at home.
- Strong communication, organization, and clinical reasoning skills.
- Valid driver's license and reliable transportation.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Perform occupational therapy evaluations in patients' homes.
- Develop individualized treatment plans that improve independence with activities of daily living.
- Assess home safety and recommend adaptive equipment and environmental modifications.
- Educate patients and caregivers on techniques that improve safety, function, and quality of life.
- Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team.
- Complete timely, accurate documentation while monitoring patient progress.
