About the Role We’re hiring an Occupational Therapist to serve patients in the Rockford/Freeport area, helping individuals regain independence and safely perform daily activities in their home environment.

Requirements

• Active Illinois Occupational Therapist (OT) license
• Strong clinical and assessment skills
• Compassionate, patient-focused approach
• Reliable transportation

Nice To Haves

• Home health experience preferred (but not required)

Responsibilities

• Perform in-home occupational therapy evaluations
• Develop individualized treatment plans focused on ADLs/IADLs and independence
• Provide one-on-one therapy sessions
• Educate patients and caregivers on safety, mobility, and adaptive techniques
• Recommend and train on adaptive equipment
• Collaborate with interdisciplinary care teams
• Document progress and outcomes
