About The Position

Join Our Team as a Home Health Physical Therapist PRN, Part-Time, or Full-Time - Monday–Friday | 8:00 AM – 4:00 PM Castor Home Health Solutions is seeking dedicated PTs to provide one-on-one, in-home therapy services in the Rockford area. With a steady Monday–Friday schedule and the option for PRN, part-time, or full-time hours, you’ll enjoy balance, flexibility, and the satisfaction of making a real impact—without sacrificing your personal life. Requirements

• Valid Illinois Physical Therapist license
• Strong assessment and clinical skills
• Compassion, empathy, and great communication abilities
• Reliable transportation to patient homes

Nice To Haves

• Home health experience preferred—but we’ll provide support if you’re new to it

Responsibilities

• Complete in-home physical therapy evaluations
• Create and deliver customized therapy plans
• Provide one-on-one sessions tailored to each patient’s goals
• Educate patients and families on exercises for continued improvement
• Collaborate with nurses, physicians, and other care providers
• Document patient progress accurately
