Requirements
- Graduate of an accredited nursing program
- Active LPN/LVN license
- Minimum 1 year of clinical experience
- Ability to travel within the coverage area (up to 50%)
- Ability to lift, push, pull, or support up to 50 pounds
- Valid driver’s license, auto insurance, and reliable transportation
Nice To Haves
- Home health or hospice experience preferred
Responsibilities
- Provide direct patient care based on the Plan of Care and physician orders
- Support patient goals through skilled nursing visits
- Monitor patient condition and report changes to the RN Case Manager and care team
- Educate patients and families on self-care and safety
- Document care, medications, and updates accurately
- Collaborate with the RN Case Manager and clinical team
- Help prevent hospitalizations through proactive care
