Requirements
- Graduate of an accredited Occupational Therapy program
- Current, unrestricted Occupational Therapist license in the state of practice
- Minimum 1 year of experience in a clinical care setting
- Ability and willingness to travel within the assigned coverage area (approximately 50%)
- Valid driver’s license, reliable transportation, and current auto insurance
- Ability to lift, push, pull, and support up to 50 pounds, including patient transfers and equipment movement
Responsibilities
- Evaluate patients to determine rehabilitation needs and develop individualized plans of care
- Perform initial evaluations and ongoing reassessments according to OASIS and PPS regulatory guidelines
- Implement and adjust treatment plans to ensure quality outcomes and continuity of care
- Communicate patient progress, significant changes, or concerns to the physician and care team
- Educate patients and caregivers on activities of daily living (ADLs), adaptive equipment, and home safety
- Assist patients and families in obtaining necessary equipment, including custom splint fabrication when appropriate
- Provide instruction and support for home therapy programs
- Collaborate with interdisciplinary teams to ensure the highest quality patient care
- Supervise and provide clinical guidance to Certified Occupational Therapy Assistants (COTA), Home Health Aides (HHA), and other clinical staff
- Ensure documentation and care delivery meet professional standards and regulatory requirements
