About The Position

CareAparent is a local and independently owned Home Health company serving seniors who want to continue to live independently and safely at home for as long as possible. Every service we offer is a response to the needs and goals of our clients and their families, and our efforts as a company are inspired by the desire to explore possibilities for meeting those needs. CareAparent is seeking new graduates and experienced Occupational Therapists (PT) to perform home health visits on a part-time basis. As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary that provides rehabilitative care to clients, allowing them to live independently and age in place safely. At CareAparent you will work with other passionate individuals focused on giving the best care possible for our clients. Our Care Management Model ensures that all team members work together to service our clients. Teams meet on a regular basis to answer questions, provide support, and stay connected. Our OTs frequently travel throughout either the Twin Cities metro area or St. Cloud metro area from one client’s home to another.

Requirements

  • Graduate of an accredited Occupational Therapy program
  • Current and valid OT license in Minnesota
  • Ability to work flexible hours, including weekends and on-call rotations
  • Valid Driver's License and reliable vehicle
  • Demonstrated ability to work autonomously

Responsibilities

  • Evaluate clients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
  • Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
  • Evaluate home environment for hazards or barriers to more independent living.
  • Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
  • Continually assess and revise the occupational therapy care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
  • Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
