About The Position
Empath Home Health, a member of Empath Health, is seeking a caring and motivated PRN Home Health Occupational Therapist (OT). The Home Health Occupational Therapist provides exceptional care to our clients in Lee County (Fort Myers) area. Empath Home Health delivers high-quality, outcomes-driven care trusted by patients, families, and healthcare professionals. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.
Requirements
- Bachelor Degree in Occupational Therapy.
- Florida licensure in occupational therapy.
- Minimum of one (1) year experience as a licensed Occupational Therapist.
Nice To Haves
- Home health care experience preferred.
Responsibilities
- Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
- Performs initial and on going assessments to determine level of functioning and patient’s ability to perform activities of daily living, including OASIS assessments at appropriate time points.
- Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self-care.
