Requirements

  • Graduate of an accredited Occupational Therapy program
  • Current, unrestricted Occupational Therapist license in the state of practice
  • Minimum one year of experience in a clinical care setting
  • Ability and willingness to travel within the assigned coverage area
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including patient transfers and equipment movement

Responsibilities

  • Evaluate patients to determine rehabilitation needs and develop individualized plans of care
  • Perform initial therapy evaluations and ongoing reassessments according to OASIS and PPS regulatory guidelines
  • Collaborate with physicians and care teams to ensure quality outcomes and continuity of care
  • Communicate significant findings or changes in patient condition to the care team
  • Educate patients and caregivers on activities of daily living using adaptive equipment and therapeutic techniques
  • Assist patients and caregivers in securing necessary equipment, which may include custom splint fabrication
  • Provide instruction and support for home therapy programs
  • Counsel patients and families on therapy goals and care needs
  • Supervise and provide clinical direction to Certified Occupational Therapy Assistants, Home Health Aides, and other clinical staff
  • Ensure documentation meets professional standards and regulatory guidelines
