About The Position

We are seeking an experienced Occupational Therapist (OT) to provide skilled home health services to patients throughout the West Portland area and surrounding communities. The ideal candidate will be comfortable working independently in the home setting while delivering high-quality rehabilitative care.

Requirements

• Active Oregon Occupational Therapy License
• BLS Certification
• Minimum 1 year of Skilled Home Health experience required
• OASIS experience required
• Home Health Occupational Therapy
• OASIS Assessment and Documentation
• Patient Evaluation and Treatment Planning
• Activities of Daily Living (ADL) Training
• Functional Mobility Training
• Home Safety Assessments
• Adaptive Equipment Recommendations
• Patient and Caregiver Education
• Care Coordination
• Electronic Documentation
• Documentation Systems

Responsibilities

• Perform occupational therapy evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide therapeutic interventions to improve patient independence and safety
• Complete OASIS assessments and required documentation
• Educate patients and caregivers on treatment goals and home safety strategies
• Coordinate care with physicians, nurses, therapists, and other healthcare professionals
• Maintain accurate and timely clinical documentation
