About The Position
Home Health Occupational Therapist (OT) Per Diem | San Francisco or San Mateo County — Dedicated Territory Assigned. At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we've served the greater Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. We are hiring Occupational Therapists for both our San Francisco and Peninsula teams. You will be assigned to one dedicated territory — SF County or San Mateo County — so you can build familiarity with your patients and your area without being stretched across both.
Requirements
- Current, active California Occupational Therapist license (required)
- Valid California driver's license
- Strong organizational skills, clinical judgment, and a compassionate communication style
Nice To Haves
- Minimum of 1 year of home health experience strongly preferred
Responsibilities
- Provide skilled occupational therapy services focused on ADLs, adaptive equipment, and functional independence
- Develop, implement, and manage individualized plans of care based on physician orders
- Report changes in patient condition and collaborate with physicians, case managers, and the Director of Nursing
- Maintain accurate, timely clinical records using AI-supported documentation tools
